Can anyone help me with some directions on how to fill out T-20-E-1 Request for Conditional Release for Soldiers with MOS?

So the T20 E1 was a checklist. That is what you build your packet with.

The 4651 has a regulation at the top, it will tell you in AR140-10 what is your responsibility as the reservist and what is the responsibility of others like the approval authority.

The checklist is self explanitory, just gather the documents listed.
Do you have the DA 368 (section 1-4) completed?
The DA 4187, TO/THRU you may need help with, FROM is your current assigned unit. Blocks 4-6 is your information. Block 8, select other and type 'conditional release'. In block 9 say why you want to switch in remarks.)
The DA 4651, fill out section 1 with your current information. Ask your S1 for your PQR if you don't know all the blocks.
DA form 4856 - counseling with commander and another one with retention.
After you and the recruiter sign the DA368 you turn it into your chain of command to get approved by the first general officer.
If/when the GO signes, you go back to the recruiter and sign a contract. You will need to provide the contact back to your reserve admin to get you discharge orders.
