There is no specific format for a QMP letter to the president of the board. However, refer to AR 25-50 when you send a letter to the board.

HRC has one posted to the promotions section. It outlines the format that is to be used but not the body content, that is at your discretion.However the LTB should be used to impart pertinent information or discrepancies concerning your records. I wrote a LTB, I got picked up first look and I feel as if the letter helped.

I recently received a QMP notice for a 4 in part IV of NCOER in 2014, all due to butting heads with a female CPT that was my senior rater. Reference DA PAM 623-3 pages 137 thru 143 everything you need is in there. Example memo's, IE sample formates for 3rd party support requests, Appeal request etc.

It's just a formal letter. I did a letter for a friend's MRB once. She is still in almost 2 years later. I wrote my own up for a vacancy I was applying for too.
