Do you need a profile to wear a compression sleeve in the Army physical fitness uniform?

I would say yes, as it is not an authorized uniform article

12–2. Composition
The IPFU consists of the following components (see figs 12–1 and 12–2 for variations of the IPFU):
a.Jacket, running, gray and black.
b.Pants, running, black.
c.Trunks, running, black, moisture-wicking.
d.T-shirt, gray, short sleeve, moisture-wicking.
e.T-shirt, gray, long sleeve, moisture-wicking.
f.Footwear.
g.Socks.

12–3. Accessories
The following accessories are normally worn with the IPFU.
a.Gloves.
b.Fleece cap, foliage green (see para 4–10c).

I'm going to say yes since a compression sleeve can be viewed as a medical device.

Absolutely anything that is added while in uniform must be documented on a profile. So it's as easy as asking you primary care provider to give you a profile for it and you will be ok.

Now, if I can find some white compression socks without logos...

OTOH, I've seen the 82nd Airborne Division HHC commander wearing compression sleeves right in front of the DCSM, and the CSM didn't say anything. An (aside from the Old Guard), there's nobody more uniform-Nazi than the 82nd Airplane Gang.
I've also seen a soldier wearing compression sleeves right in front of the ARCIC CSM (now employed as a troubleshooter for SMA Dailey), and he didn't say anything either.
