Since Charles has been retired for 22 years and form numbers/editions change over time, he can&#39;t suggest a form number. Things that he knows will be required are the full account of events from the individual who is still living. Have that person write it out or have someone do it for him. Have him sign it with his name, rank and unit. Additionally, locate anybody in the command chain who has any knowledge of the event(s) and get similar statement(s). These don&#39;t need to include all of the details, but the more details, the better. Also include any other supporting documentation that the form requires. For example, her uncle&#39;s discharge documents showing that he was KIA. <br /><br />A good place to start would be by contacting one of the individuals or families who have recently received extremely overdue awards. Learn about the process they went through to get their award(s). Guy had 2 or 3 silvers, bronze w/clusters, and SEVEN purple hearts! His co in 44, a major, had prepped the recommendation for moh, but was then killed! Paperwork lost, found in 70s, awarded moh by Carter in 78! Can&#39;t remember his name, though! The point being, it can be done!
