Does anyone have an example 4187 to disenroll from MACP?

SITUATION: I am currently enrolled in MACP; the husband and I are out of FT Hood. He is less than 3 yrs from retirement and does not want to leave FT Hood. I, on the other hand am trying to get to the RNCOA at FT Gordon for career advancement. Ergo, I will be going to Gordon alone. I just need an example 4187 ASAP to send to him for his signature so I can get put on assignment to Gordon without pulling him from Hood. My S1 is having no luck...

SSG Private RallyPoint Member Which MACP are you referring to? I kinda assume it's the Modern Army Combatives Program?
