Does anyone know how to get a DD Form 1A?

I accepted a direct commission in January 2018. The appointment memo said I would receive a DD Form 1A (Officer Commission) in the mail but as of today it hasn't come.

http://expressmilitary.com/dd-1.htm This site came up to order it while searching as it is a controlled form in DOD. Looks like a commercial site I HAVE NEVER USED. Not something you really need as I never received one for 2LT and I do not think I have one for 1LT either. Good luck.

Your BN or BDE S-1 could replicate one for you, I’d speak with them and provide them a copy of your DA71 to validate the date, time, branch and rank.

I think a heart felt congratulations is in order. Do your best to listen to your PSG and learn. Their job is to train you up to become an outstanding Capt./Company Commander. Yes, you are the officer and he/she is aware of that, just listen and learn something. They will not steer you wrong and besides you probably know when someone is full of BS. Good luck in your career in the wonderful life we all call the US Army! When you are ready to resign your Commission try to leave it a litter better than when you signed up and God Speed!

So here is the actual answer.

How can I obtain a DD Form 1A?
A: AR 601-100, paragraph 2-13.g.(4) states that, “On receipt of DA Form 71, the relevant Army command, ASCC, or DRU commander will issue DD Form 1 (Officer’s Commissioning Certificate) to the officer.

If an Officer did not receive their commissioning certificate, they can contact the Reserve Appointments Branch, Human Resources Command - Ft. Knox at [login to see] for assistance.

If you have your commissioning order, you can go to your nearest Army installation and they can prepare one for you

I was a direct commission too. My appointment memo never said that. Maybe because I was a direct commission in the chaplain corps branch in the reserves?...Beats me. Did you ever end up receiving yours?
