She is a decided asset.<br />As a clinical psychologist and professor, my main concerns are related to Behavioral health Care.<br />I continue to see in my clinical consultations w/ combat vets back to Korea, unresolved combat trauma and compacted grief. I also firmly believe in group therapy over time to be an efficacious intervention as it recapitulates the intimate psychosocial unit: the squad. I think GTH. is underutilized and often conducted over 6 weeks in many circumstances in the most urgent vets.<br /><br />How will this additional care track help reduce the 20 suicides per day when only 6 of 20 are not being followed at VAC&#39;s? I have been denied mental health claims since 2002 which I should NOT have been denied, only because the v.a. claims that there are no records which indicate I deserve mental health care. I have been given mental health treatment only sporadically and with no care to my needs based on people not believing what I say and calling me a lying prick and worse.<br />Because I have been denied mental health help, I attempted suicide MANY times and even got sent to prison due to being treated like shit.<br /><br />I am completely done (after 19 years of abuse) playing nice and having tact. I want what I deserve and need to move on with my life and get out of this shit apartment I live in. I want a damn job, I want a life. And I damned well want the dental care I need so I can stop being forced to perform dental surgery on my damned SELF, just because I incorrectly rate 30% and not 60% or higher to qualify for dental services.<br />I am tired of being poor, living on so little money, and living in an apartment with horrible management. But I built my company to help my fellow veterans, active service persons and their families. Yet, this stated group is the hardest for me to serve.<br />As a Texas Medicare IDTF #533293 (ultrasound only), Medicaid/CHIP, TRICARE, BCBS and Atena IN-Network provider, I have reached out to all veterans groups in my area and I have joined the American Legion to shake hands.<br />I do not know for sure, but I get the impression most of these older veterans would rather travel the 45 miles to the Dallas, TX. VA for thier ultrasound imaging as well as wait 15 to 45 days for their exam. I may not be seeing every angle of this situation. <br />Does the VA have a data base of providers that work along side the local VA?<br />As a VA certified veteran small business, I wrote a proposal to the VA. Being only local is an issue. Also, it appears those that hold the VA contract to provide imaging do not want to lose any patients. Travel distance and wait times be dammed?<br />Thank you in advance for reading my post. Any thoughts you might have are welcome. Most of my care is through the Charles A. George VAMC in Asheville, NC. I use Community Care for mammograms &amp; hearing aids (due to wait times) and use Medicare/TFL for my sleep apnea (wasn&#39;t happy with my care at VA). I participate in the VA MOVE program &amp; am in the VAMC with some regularity. I have experience with practice management in this region.<br /><br />With the transfer of CAG&#39;s fantastic director to the TN region &amp; the prolonged time with an MD as acting director followed by the implementation of the Mission Act &amp; its emphasis on community care &amp; whole health, I see an erosion of quality care for Veterans in general.<br />For example, my hip replacement was done by a Duke Medical Resident under the supervision of a very senior orthopedic surgeon. Now, CAG has no residents, partially because of the schools but I wonder if the VA administrative changes are affecting it as well.<br />Can you promise that we can move between Community Care &amp; VA seamlessly without jeopardizing the continuity of care or our personal information/medical records? Veteran requests prescription be refilled August 8. Veteran runs out of medication on August 14 (30 days after original refill). VA fills prescription on August 15 (because it was filled on the 15th the month prior, so they won&#39;t fill it prior to the 15th regardless of the number of days in the month), VA provides it to UPS on August 17. I finally, called again and was a little more assertive and was told she had a lot of referrals and that I would have to go to only one place in Dover De. (closest) to me, otherwise drive over an hour close to the VA hospital, which I thought this program was suppose to alleviate. I went ahead and am using my outside insurance since the VA can&#39;t get me in, in a timely manner. <br />Also, have had 2 arthroscopic surgeries on my left knee and need to be seen due to issues with it and they can&#39;t get me in til the end of August, again is that a timely manner. This program is not helping me so far in any way, I am not able to see a provider that would be a good orthopedist to take care of me. Which then, as I understand it, would not be reimbursable.<br />Related question: the procedures I&#39;ve seen suggest that you need prior approval to get reimbursed for a visit to one of these non-VA locations, so how would that work if I, say, fell down and broke my leg? I requested to see a dermatologist back in March 2019 when I was told that I first needed to see my primary doctor which they scheduled 3 months out as that was the first available appointment. I ended up receiving a random call from them a couple weeks later saying they had an opening within an hours time with another doctor who could approve my dermatology appointment. After seeing the other primary doctor, I went to make an appointment with a VA dermatologist and was told it would be a 4 month wait. I requested to use the Veteran&#39;s Choice program, now called the VA Mission Act, and was able to get an appointment in about a month due to my traveling. The private dermatologist prescribed me some medication which I brought to the Santa Rosa VA Clinic to get filled. The pharmacy told me that they could not approve the medication unless I saw a VA dermatologist first.<br />So my question is this, what is the point of being allowed to see an outside private doctor utilizing the VA Mission Act if the VA won&#39;t even cover the medication that is prescribed? Now I am forced to wait to another 3 months before seeing the VA dermatologist which I could have set up back in April and had an appointment in July, but now must wait until September. We faxed over to Randy Bunting, NSO, all the paperwork we have from several doctors, proving that I have permanent injury to my right arm because she had scraped and moved the nerve in the said arm. Other VA Center such as Augusta allow their patients to go to a Chiropractor. When I heard this I decided to try it out and stop taking so much medicine. The VA paid most of the bill but one company continues to bill me for radiology. When I asked the V.A. about this, they said to point them to the V.A. and they would handle it. Since then, I have continued to get bills and have gotten three different answers from the the V.A. with the last being that the medical service was not authorized and they were not obligated to pay. I am trying to find an answer on why I have gotten so many different answers, pointed to three different departments and finally gotten an answer that is 180 degrees different than the original. I have two other issues to ask about but this could go on for hours. It seems that there is more confusion and gate keeping at the V.A. than there was before the Mission Act. And the Mission act over Community Care... whew? This might have had good intentions but it actually looks like a wall built to keep veterans from getting needed care that the VA won&#39;t or cannot provide. Alcoholism, without question this disease is a contributing factor in these suicides (25 to 40%) and it goes on seemingly without notice. There is a solution and for many veterans it is and has been Alcoholics Anonymous, I am an alcoholic with 16 years of sobriety and I am presently working with other alcoholic veterans as well as a few active duty military. <br />I have requested that I be allowed to set up an AA meeting at the local Veterans Hospital, my original request was made in late January of this year and while I still have hope there is no resolution in place at this time. <br />There are no costs related to this request, just a meeting place for 1 or 2 one hour meetings a week. I will be working jointly with other sober alcoholics whom many of including myself considered suicide as a way out, but we are here and offer a solution to others thru the veteran hospitals and clinics. <br />There are many that wish to help and there are no costs involved with our service as we do this in an effort to stay sober ourselves. The only place where I could get further health care was Canada where I was admitted to Lynhurst Rehab Centre for my condition for 2 and one-half months where I learned how transfer etc. Because I taught up in Canada for 34 years after VN service I had health care provided even though I was living in California for 10 years. I was given many years of excellent health care by the VA in the Palo Alto VA but they did not provide health care when I really needed it. The first seems to be pretty much ignored by the VA and the 2nd seems to only get treatment to mask the symptoms such as cymbalta which only allows it to continue to get worse with less recognition. Why are some of the newer and unique treatments to reverse Neuopathy not recognized by the VA and available to veterans since reversing the damage could save millions on continued RX masking and eventual falls, amputations etc. and allow a much better quality of life? And secondly when will the multiple concussive brain injuries be recognized as a serious and crippling disability and some of the excellent new treatments and realistic compensation for these ailments be addressed? I walked in, and it was standing room only, so I walked right back out.<br />When I asked if there were other events scheduled, I was told that the VA didn’t fund rollout, that the DAV had to find a free room, not the VA, so if I could find a free room she’d be happy to do it again in a larger venue.<br />She said no one else said anything, and I asked her about tunnel rats, and other people with difficulties in being in crowded rooms, folks like me that self selected and so she never has any contact with them. Crickets.<br />They stopped sending information mailers because they cost $3,000, and, “(Veterans) would just throw them away”. Rural Arizona is vast, and many homes are off grid, so I asked her about that again. Crickets again. It’s like city dwellers can’t fathom what life is like out here, and don’t care to.<br />I can’t talk on the phone, so she insists that I do.<br />And 20 Veterans a day die by their own hand while this goes on.<br />The doctors at Prescott are great, admin is very lacking.<br />Can you please fund actual help, and please don’t use the patient advocate, they are part of that administration, and the tip of the spear in, “sorry, not my job”, “it’s not our fault”, advocacy? Maybe go back to Congress and say, “Oops, we forgot to fund this part, we didn’t anticipate that there would be this much interest”, I don’t know.<br /><br />2) The VA talks a great game about mental health, but don’t actually have the services they advertised. “Same day” services consist of the suicide hotline. I had a low point back in March, asked for help, but you have to talk on the phone. I finally got through via typing, but since I wasn’t interested in suicide, I became less, I don’t know, maybe interesting, or whatever, but it was obvious that she had other things to do.<br />I talked to Prescott MH (only during normal business hours, they have an answering machine for night and weekend crises), and asked for mental health Community Care. That was in March. They say they’re working on it, don’t know what happened to it, etc. I understand that your first question will be if I know about the Vet Center. Yes, but there’s a long backstory, i was attacked there, I can’t go there anymore.<br />Oh, and if the counselor that they choose isn’t compatible, I have to restart the entire process, they can’t do a backup. I’l fall into the crack and might not even try again.<br />So I wait for a counselor.<br />Can you please help with getting a Community Care counselor?<br /><br />3) Do I have a copay? I’m 100% U/I, never had a copay before, but the VA stresses that there’ll be a copay without saying for whom, but no one can answer the question.<br /><br />4) I got hurt in June of 2018, my back and hips. It takes months to get an X-ray, I need a consult for each hip individually, and my back, all as separate entities, no way to get into a doctor and have him see what needs done, because there’s no authorization for the problem as a whole. I see one doctor for my left hip, another one for my back, and now there’s a new consult hopefully floating around out there for my right hip. It’s ridiculous. Can you please set up so the Community Care can cover more than one body part at a time?<br /><br />(Edit) My NP called, we apparently don’t have any doctors here that treat both back and hips, but they’re getting me in with the same hip doctor as my left hip. When I started using this clinic almost nine years ago I have been complaining of neck and head problems which have progressively gotten worse. Finally, about six months ago, a radiologist and physical therapy technician, (not the primary care doctor) found what is causing my problems. Finally having a diagnosis I thought I would get the help I’ve been needing for so long, but here it is six months later and I still have not seen an ENT doctor about my condition despite asking for help on a regular basis. My question is; given the clinics inability to provide me with effective healthcare, and a secure effective primary care doctor, can I qualify for the use of a civilian primary care doctor under the new Community Care Act? The clinic has systemic, consistent problems that have not improved in almost a decade and needs the be put under a microscope. If I am having such problems I cannot imagine how my fellow Veterans with more serious problems are being affected. We provide orthopedic care for veterans through the Patient-Centered Community Care (PC3) program, which works like a HMO insurance plan for civilians in that medical care must be authorized by the patient’s primary care provider (PCP) prior to being seen by a specialist. Within the scope of that authorization are numbers of authorized visits (generally not more than 3 visits for civilians and 6 visits for VA patients) as well as a valid period (generally not more than 3 months for civilians and one year for VA patients).<br /><br />Overall, this is a great program for veterans (both retirees and non-retirees), as they pay nothing for prior authorized care, but I have one major concern.<br /><br />It seems like a literal act of Congress when continued care is needed for our veteran patients that extends beyond the initial valid period of authorized care. If I send a request for additional services to the VA to extend the valid period of authorized care, it is never approved in a timely manner. I can even give as much as a 6-month lead-time, and yet will be told that my request is still pending approval. Compare that to civilian PCP’s. When I send a request for additional services to a civilian PCP, the average turnaround time is less than one week. Some will even have my request completed in less than 24 hours.<br /><br />My question is will there be a fix for this. As of right now, I have several veteran patients who are in need of orthopedic care that extends beyond the valid period of their initial authorized care, but we cannot provide such care under the PC3 program because we are waiting on VA approval. The VA has indicated that I must prove medications offered by them do not work before they would offer to cover my proven effective medications. Why do I have to take the physical risks of dropping a proven long term treatment to experiment with other options? Makes no personal health sense to take such risks. The reason that I am writing to you is because I have a concern about the policy that the VA has against the thousands of veterans like myself about dentistry care which a veteran has to have eighty percent or more disability in order to receive free dental care. I have spoken to the VA about this matter and was told that I will have to buy my own dental insurance and to be truthful, I cannot afford to pay for it for I’m on Social Security, unable to work and on a strict income. Looking at the cost of dental insurance and what it covers can be overbearing with co pays and if you need surgery, the cost can be in the thousands. Sir, I have talked to many of veterans that are in the same shape that I’m in and the conversation always ends the same, “We cannot afford it”. It is true that the VA will Give me care from my toe to the top of my head regardless of what my problem may be, but when it comes to my teeth, I have to be eighty percent in order to get care. With my teeth becoming rotten and falling out, a simple smile is not something I can do. I believe that it is unfair to the thousands of veterans that are experiencing the same problems that I have without dental care from the VA which should be free to all honorable discharged veterans. I wrote a clinical appeal 2 weeks ago and havent heard anything back. I am trying o get them to renew my doctor visits and get the pharmacy to give me my meds. It took them a whole year just to get me 10 visits for community care, when Im supposed to be seen every 2 weeks. Community Care was used in May 2018 to enable me to see a local dentist. Fantastic for me, but not for the local dentist. The VA still hasn&#39;t paid this dentist for my authorized exam, x-rays and cleaning on 07/06/2018. The dentist and I have tried several times per month since that date to get the AtlantA VA to pay this dentist. Atlanta VA Community Care employees for dental care do not answer their phones and do not return their voice messages. My VA dentist and her superior can&#39;t get anyone to respond either. The dentist has sent the treatment plan and billing statement by fax and by mail countless times. I&#39;ve been in his office and watched the forms being sent. I know it&#39;s been done multiple times and correctly as required. How can I get the Atlanta VA to pay this dentist?<br /><br />2. My VA dentist also submitted and her boss authorized 2019 Community Care dental care for me more than 1 month ago. Neither the dentist nor I have received any notice for scheduling an appointment. What can be done at the Atlanta VA to get the 2019 Community Care processed?<br /><br />Also, my VA dentist submitted and her boss approved one week ago, an emergency Community Care request for an abscessed tooth that I developed. They did this immediately for me. However, neither the local dentist nor I have heard anything about scheduling this emergency appointment. What is not happening at the Atlanta VA?<br /><br />3. For other Community Care needs that are not dental, the consults are being processed efficiently. Also, Tri-West representatives are outstanding--curteous, respectful, kind, most willing to help, and efficient.<br /><br />4. I have the deepest gratitude to President Trump and the team he assembled to design and implement the Mission Act. It is a blessing. I can now have an appointment with a local cardiologist. It&#39;s taken 3 years of trying to get the Atlanta VA to approve local cardiology care for me--neither VA Choice nor Community Care would be approved for me by the Atlanta VA. I kept reporting my shortness of breath and the number of times I&#39;d collapse and it was to no avail. I was told I&#39;d have to go to Atlanta or I wouldn&#39;t get medical care. My condition has worsened to the point I can&#39;t walk to my mailbox without collapsing from shortness of breath. My wife can no longer get me up off the ground by herself. Neighbors must come to the rescue. So, if it weren&#39;t for the Mission Act, I&#39;d never get cardiology care from the VA. (Recall that ATL is 122 miles from my home, I do not have transportation to get there and the Atlanta VA would not help. So, THANK YOU PRESIDENT TRUMP! We have been providers for the VA for years but the new Community Care portion of the VA.gov website does not show us as a provider. It shows other providers that are 60-90 miles away. I called someone in your office and they helped me get in touch with TriWest who said we were still in-net with the Certified Provider Program but that we have to start over and request to be in the PC3 network. <br /><br />Here are my questions:<br />1. Why would they not transfer our information to/from TriWest to make it a smooth transition instead of leaving us off the website and making us register all over again? This benefits no one...especially the Vets!<br />2. The Vets in our area that have used us in the past WON&#39;T go anywhere else. They want to see us and we care about them. We are attentive and do our best to provide the best for them in a timely manner. With that being said, what do they have to do when they want to come see us? Do they have to call someone to tell them that they are 80-90 miles away from the nearest VA facility? Do we, as the provider, need to call someone to tell them they should be eligible for CCN?<br />3. Who do Vets and Providers contact at the VA in order to get authorization for them to see and be eligible to see us in the CCN?<br /><br />The last thing is that the Community Care Network should be more prominent on the VA website. Per the VA you are already in the same position I am. Though not an employee I was 40 years ago a volunteer, and applied again about 6 or 7 years ago. Not hearing back I dropped the issue. Now that I&#39;m retired and having used the Veterans Choice program for the past 3 or so years. Three weeks ago I had to see my doctor. So I called the advice nurse, whom stated I needed to be seen with 2 to 8 hrs. Yet, there were NO same day appointments. The earliest wouldn&#39;t be until Monday the following week. Yet, (here&#39;s where it gets worse) the nurse says I&#39;m eligible to go to Urgent Care. They give me the phone number locally. Wow the clinin is less than 1 mile. I go and check in, presenting my ID. The clerk says she needs to call to verify I&#39;m eligible. I said the VA told me I am and to come here. See I&#39;m also 100% Disabled. Getting ready to see their doctor. The clerk says sorry you were denied! That can not be. Still on the phone I speak with the TRICARE agent. Whom stated no as of May 27, 2019 my UC was coded with the expiration date. This started on July 23rd, 2019 at 0645. By 1600 I was called by a supervisor from TRICARE and the VA Advice Nurse in a conference call. <br />Told this would be fixed by the following day. That in the meantime they were advising me to seek care through the ER. Since the closest VA ER was 80 and 50 miles away. <br />Fast forward to today August 7, 2019. Still has not been corrected. I have filed complaints with White House Counsel, VA OIG and a Congressional. Since everytime I call either agency. I&#39;m still on the Marry-go-round. VA points the finger at TRICARE and vise versa. Speaking with the Congressman office there seems to be 3 senereos. One I have to fix the date? WTF! 2. They are not sure how to merge these two files 3. My favorite Do not get sick. <br />As I told the Congressman office. My next contact will be with the News agency&#39;s. <br />Asked what do I do WHEN I am told to seek care at the UC clinic? Do i pay and get reimbursed? Go to the ER instead? Suck it up? This was their answer........I Don&#39;t know. <br />If this was a movie no one would believe it.
