I talked to finance and they told me to talk to my S1 to get it stopped. My S1 simply told me that I needed a 4187 for it, but did not know what it needed to have on it. In my opinion (as I was not an admin or pay type person), check the "Other" block in Section III and put "Stop FSA."<br /><br />In Section IV, put the reason.
