Example DA Form 4187 to stop Family Separation Pay?

I have a soldier who is receiving family separation pay that should not be. I talked to finance and they told me to talk to my S1 to get it stopped. My S1 simply told me that I needed a 4187 for it, but did not know what it needed to have on it. Does anyone know where I can find an example of this as I'm not finding anything.

Did you ask S1 what to put on the 4187?
I looked up the SF 4187. In my opinion (as I was not an admin or pay type person), check the "Other" block in Section III and put "Stop FSA."
In Section IV, put the reason. The rest of the form should be self-explanatory.
