Check out this link, it has to do with Iraq and Afghanistan Campaign medals. Apparently even if you deploy only once, you need a campaign star...

Upon reviewing a few dozen records, this was a common error.

Every Iraq or Afghanistan Campaign Medal (and it's ribbon) must have AT LEAST ONE campaign star affixed.
