You will need the application, supporting documentation, and a DA 4187. Please do yourself a favor and familiarize yourself with Chapter 6 of that regulation. Make sure to have several affidavits and if it involves custody make sure to verify sole custody before application. I am currently dealing with this so I am just warning you make sure it is specified in Reason For Characterization on your Discharge and DA 4187 as well as what is causing specific dependency. Make sure to verify a solution, you are only one able to alleviate, describe what you have exhausted to come to this point. Make sure that you prove permanency rather than temporary.
