How can I get a Memorandum for Record?

I am trying to reenlist into the Air Force. I was discharged from boot camp from MCRD Parris Island due to a fractured femoral neck. My RE code is RE-3P. Everything was going smooth until the recruiter told me I needed a surgical release form. I was advised three different ways to advance through the process.
1. Wait for the form to come through.

2. Get a memorandum for record and provide updated x-rays and a report from a physician. I have no idea who I would contact to get the memo?

3. Try to upgrade my reenlistment code. Since there was nothing wrong with my DD-214 and I am just trying to upgrade the code I do not know if I should contact the Board for Correction of Naval Records (BCNR) or the Naval Discharge Review Board (NDRB).

Since I am posting here option 1 is not on the table. I have x-rays and a physician's report stating I am healed and there are no abnormalities. Just trying to make sure I choose the best route going forward.

The memorandum for record is just a memo written by the physician that states you are healed.

Look up a site called eVetRecs, of NPRC the record center of NARA, in St Louis, there isn't much space in the Java applet to rill out and send in, somtoull likely have to abbreviate, ask for both your personnel file, as well as your medical file, the stuff might nenthere by now, depending on how long ago you were at Parrots Island. Print the Java applet response after the request goes in online, sign and date the form, attach a cover Petter of you need more room with any other stuff you'd want to send them about all you're needing to do what you're seeking, so they comprehend the problem...

Don't lose the formz it's got a reference code/number you'd need to give the phone staff, look up the main phone, one can call the senior staff if need be, it is poswibgle, however, the phone bank staff is quite good, if you've got the reference, sign and date the Java applet printoutz no signature, no date, they can't do anything from your submission using the Java applet on the eVetRecs websotez they'll see it, they can do nothing without a signature, just make clear you need your personnel and medical field, they both have to he explicitly mentioned separately....

nPRC will have your stuff, NARA won't, there's a till free for NARA as well as DC offs you can call, all they can do is get you ahopd of the NARA senior staff....

If you submit a valid request for both files, you'll get a letter in a few days they're doing it at NPRC, it takes a good month or two, plus, your stuff might not all he in St Louis yet, they have svc reps there from alp svcs, all stuff is in what they term different reference cores, they just moved into a new bldg a few yes ago...

You'll year about a fire in the 70s there, that only affected Army records, not yours, so don't worry about that, it was in their old bldg now long gone, I gather....

Their phone is 314 801 something just look it up using Google or call information, youmcam get it, trust me, I've done it sevl times for my stuff and my family stuff, relatives, my uncle, who'd been WW2, and my wife's family stuff for my mother in law we needed it to try for VA survivor benefits for her, using mymwifes Dad's, stepdad's, and broehr's stuff...it takes awhile, so you've gotta he patient, remember the site to search for is eVetRecs, trust me, that's the right one, promise, OK?

If you were ever in Amy VA hospitals for anything, that stuff isn't through eVetRecs, you'd have to go to any VA Hosp ROI off, release of infoz their med record offices, to get your stuff, VA main won't have it, they could get it, however, each VA Hosp keeps its own, I've gathered...all svc stuff is held by NPRC, I've gathered as well, however....

Your biggest issue is the P. Permanent are hard to waiver as one physician is basically overriding another. Your documentation and the memo provided by a physician, I would say an orthopedic specialist whose focus is Femoral Fractures would be where I would go for a letter. Forgive me if I stated the wrong field, the point is that it need to be a specialist not a family practitioner. Once that is straight you stand a good chance with recruitment.
