I was told that I need to put together my IDES packet to turn in to start the process. I&#39;ve filled out everything I feel comfortable filling out but now I have to fill out this DA FORM 7652 (Disability Evaluation System Commander&#39;s Performance and Functional Statement. I don&#39;t want to mess this up. I don&#39;t know why my CO can&#39;t do this. I don&#39;t have the ability to properly word this document. I&#39;m on a permanent profile due to the fusion and sciatic nerve damage. Could someone please help or point me in the right direction? It would be greatly appreciated.
