How do I receive Dr Mathews session?

I read most of session. I was not impressed by her limited replies. My question which I never got to present is: How do you get community care approval? I have been to see my PCT but the Dr answer was we can handle it all you do not need outside care. I have spent hours, yes hours, on phone trying to get approval under the time/distance provision but NO one wants to talk about it or the "grandfather clause"
