How do I tell my PCM to put me on a permanent profile?

Long story short, I’ve been dealing with the same back injury since June of 2020 and have had my profile extended every month. My pain management doctor just extended it 90 days causing me to be dropped from ALC for the 4th time. I have to attend ALC prior to December which is when I can be boarded for E7.

What do I need to do? I know the events of the ACFT that I can take and am able to do the mandatory events.

Request an MRI to ensure there is no underlying issues( compression of disks or fractures)

Ask your PCM for a P2 and chances are most will do it.

Most locations have a policy that you have to exhaust pain management and physical therapy prior to being placed on a permanent profile. Tell your PCM you're ready for a P2 and you no longer want any more temp profiles. Have the same conversation with pain management as well

You just go talk to your PCM. I was at an Air Force Base when I got on permanent profile. AF civilian doc. I told him how to fill out the form (old profile form at the time).

When I had to go to SLC I had to have Ft Riley transfer it to the new one. I just sent them the one I had. They put it in.

For the ACFT our PA updated everyone's profiles based on what we already have.

All you can do is tell your PCM you need a permanent profile and go from there.

I asked my PCM to schedule me fit for duty physical. After the physical, I was deemed “unfit”to continue service. The medical retirement process started shortly after. That’s what you want right? Medical separation?

Just ask your PCM after explaining the nature of the injury (if you now have a new PCM that is unfamiliar with the case), explaining the recovery route taken. Then....just ask. That's how I got my P2 for my feet.

I would really be cautious of this. When she go so many days on profile you will trigger a medical review and potentially a med board. In my last unit they were very active and would recommend Med boards very quickly to separate people.

Most pcms will work with you if you are straight with them. I knew what i was capable of and knew surgery would just make things worse, rather deal with the pain and stay in the military. I had a very frank conversation with my pcm and physical therapist. Both agreed and helped me craft a permanent profile that allowed me to protect myself but still kept me deployable.

And how many times have you been dropped from ALC? And why were you dropped before? Were you dropped prior to attending or during the course?

One thing that I do not understand, why is it that most of the complaints about medical issues are not from Combat Arms soldiers but the high speed low drag rear echelon types?

Go to med board to get permanent profile. But you risk separation.

Back issues since Jun 2020, that's not good. I forgot what the regulation say word for word, but if you keep going back for the same problem up to 3x, they can put you on permanent. I know for a fact they can put you on Temp profile up to a year, after that, it will be permanent. Since you said you can pass take a pass the new pt events, I wouldn't too much worry about the MEDBOARD, they can retain you even if you have to switch MOS. The Army only care about 3 things: Pass Fitness test, qualify on weapon and be able to deploy.
