How effective is the military care for those in the discharge process?

How effective is the care shown by the military to service personnel discharging through retirement or ETS? Do they truly help those transitioning out or is it little more than lip service? What has been your experience? How can the military do a better job of assisting the SM through the transition process into civilian life?

So far (not too far in) it's been very focused on big boy rules. You're going to get out what you put in.

From what I understand the VA is now included in the discharge process, this is something that has been long overdue.
