I need a tattoo waiver signed for my OCS package, even though I don't have any tattoos. Who at my place of duty needs to sign it?

According to the OPNAV instruction for enlisted applicants applying to OCS, I still need to include the form in my package even if you don't have any tattoos; it's just a way of verification I guess.
