I do not see an O-5 memo on the list of required Items in the packet.

DA 4187 (Signed by your Company Commander)
Soldier Record Brief (SRB)
DA Photo (Exception Memo is provided on website)
All NCOERs
All Academic Evaluation Reports (DA 1059s)
Last three APFT and ACFT Score Sheets (Digitally Signed by unit First Sergeant)
Army Body Fat Worksheet (DA 5500, Male) or DA 5501, Female (if it applies)
Current Physical Profile (if it applies)
Career Counselor Memorandum
Soldier Reclassification Request
One Page Essay, “Why I should be selected to become a 51C”
Security Clearance Verification Memorandum
NCOs with a Degree, copy of transcripts with conferment date and diploma only
NCOs with college credits only, copy of most current transcripts and Education Center Memorandum
Additional Documentation in .PDF only (i.e. DAU Courses, provide DAU transcripts only)
Letters of Recommendation (optional)

The LOR from O-5 is not longer a requirement. The DA 4187 from your company commander is required. However, having the LOC from O-5 could be considered in additional document portion on the website.
