I've received conflicting information in light of going from "appointed reserve" to Regular Army Commissioned Officer. AR 601–100, AR 135–155, and Army Regulation 135-100 don't say, and I'm not finding an associated DA PAM. You fill out the same side as every other commissioned Officer. The DA 71 is in need of a serious tweeking. That is where i get the right answer most times. There is a whole lot of information there not covered in DOD publications. You will stay a Reserve Officer until then. The commissioning paperwork was/is more along the lines of Command, NJP, Re-enlistment type things. I did it when I made CW2, it was pretty basic and simple, didn't really change my life. I only did one re-enlistment for a great NCO I knew.
