Should a NCO receive a "did not meet the standard" on their NCOER for a soldier that constantly didn't take care of his/her personal hygiene even when you counseled them numerous times?

There’s only so much a NCO can do. This isn’t the old days when you could gang up on someone with a hose and a scrub brush.

That seems too much if that is the whole story. That block check indicates a failure to be an NCO, and should typically be accompanied by separation proceedings, and overall will have significant career stopping/ending effects.
