What are my chances of going back to OCS if my Bn Cdr marked box 5 (definitely not recommend for suitability to come back to OCS)?

I was in Army OCS about two years ago but I got dismissed due to peer evaluations and a few SOP violations. Ever since then, I did nothing but good things. I got promoted to E5, won the Solder of the Month Board, scored high on APFT and M4 qualifications, and kept my record clean. However, in DD Form 785, the battalion commander marked box 5 (DEFINITELY NOT RECOMMEND for suitability to come back to OCS). I know that I'll need a waiver for this, but does this mean that I can't get commissioned at all? I talked to my chain of command, and they're willing to help me out.

Guard or Active?
Get a letter of recommendation or three, if you can, from ranking officers. officers in your direct chain of command who can speak on your performance.
The BN Commander's comments were only recommendations. Not a direct order.

I was getting conflicting answers for that one. Some people told me that it's waiverable as long as it's not Box 5, and others told me Box 5 is waiverable. Although Box 5 makes it harder to get a waiver, HRC people told me to submit a waiver because it's waiverable but not automatic. Fortunately, I didn't get any Article 15s.

Just understand that your name will get around at OCS. It’s a small place, even with rotating cadre. You’ll be known before you even get there so the microscope will be on you.

Yeah, that's what I was worried about. Since it's been a few years, I was hoping that all of the previous cadres and OCs left so they won't know what I did previously and I don't have to battle negative perceptions spreading around.

Can’t say I’ve been in this situation before and definitely don’t have specific advice SGT but I can say that I am in the process of dropping my OCS packet and have ran into my fair share of similar obstacles. With that being said, as the above stated the worst they could say is no. Keep trucking and take your CoC’s help since they said they are willing.

I've got all the required documents ready all signed by my COC, and it's ready to be submitted!! One thing that I'm curious is that do you guys think some of my cadres (both NCOs and Officers) got their 1059/NCOER/OER got dinged because some of the Officer Candidates got recycled/dismissed even though all of us passed the required exams?

No, the performance of students has no impact on the Cadre’s evaluations unless they did something illegal or had complaints from Candidates that were founded.

You stated it was bad peer reviews and "several" SOP violations. You got booted because 1. You weren't a team player (well, your peers obviously didnt believe you were); and, 2. You couldn't follow rules, but want to lead someone.
IMHO, it appears you still aren't ready to lead, because your comment suggests you want to blame others for your actions and behavior. It may not be the case, but you are the one who made the comment.
This isn't a personal attack, I'm just pointing out your contradictions.

1SG (Join to see) - Having been through this BS, getting gigged for an SOP violation in OCS isn't exactly uncommon. TAC's hammer you every chance over anything they can come up with as a reason and it all isn't meant to be fair or justified, just add pressure to situation. The leadership positions are rotated, get in a weak chain of command and have the TAC's dog you during the entire rotation sucks and bad peers can come out of one. Now this soldier could be a complete screw up, but his record since says he's a pretty good soldier.
I had a bet my DOT moment. The TAC's wanted me to change an Eval, which pissed me off and then questions my integrity for writing it. I just asked if they wanted to continue the conversation about falsifying an evaluation in front of the Commandant. They let me alone after that for the most part.
