<div class="images-v2-count-0"></div>1) gather as many facts as possible of the date, time, circumstances of loss/ damage, persons involved, and so on. <br /><br />Block 9 is THE most important part of the FLIPL, and needs to list The Who, what, where, how, and (if possible) why the loss/damage occurred in a logical paragraph. Use extra pages if necessary.<br /><br />Attach all available records that support the loss/damage, to include hand receipts, dispatches, PMCS records, photos, etc.<br /><br />Pay attention to the formatting of each block in AR 735-5.<br /><br />Stay within the initiation timeline - the FLIPL process starts on the day the loss/damage was discovered.
