What are the most used Navy Forms?

Army Guy Here, doing some research on forms in the Navy. In The Army we seem to have a Form for everything (Height/Weight, fitness test scores, leave, equipment hand receipts, vehicle maintenance, counseling, and so on) Looking to find what are the most common forms found in the Navy. ...and do we call those DN Forms? (DA Forms for Army...) Thanks!

When I was in as an admin officer, it was the Page 13 which was for any and everything.

It seems to me the most used navy form would be medical forms like the
DD 2808 Medical Examination Form
DD 2807-1 Medical History Forms
NAVED 6120/4 Physical Health Assessment
MILPERSMAN 1306-1200 Limited/Light Duty
S-600 Chronological Record of Medical Care, because a lot of different looking forms are called SF-600's

PHA (Physical Health Assessment), Page 2 (Dependents list), 3500 (signature page for training), MSDS (Material Safety Data Sheets) used for maintenance and upkeep just to name a few.

NAVPERS 1336/3 - The Special Request/Authorization. Used to have fill out in triplicate. But don't know nowadays, probably submit online.

You normally have official Navy forms, that are from our upper echelon (SECNAV, CNO, etc.,) and then most commands have their own forms. All Commands have one office that is over forms management, because if it doesn't have a form number it's not official. There are a lot of those out there too.

The two that I remember using where the "request chit" used for everything for requesting to take the advancement exams, schools, leave, T.A.D orders, etc. The other was the 1250's used to get parts, supplies, tools, equipment. But this may have changed since I've been out since May of 97 after 20+ yrs.

My most used form was the NavSup 1282 and the NavSup 1090. I used them everyday. Never used a counselling chit, that's what my assistant was there for.

Navy doesn't use a lot of forms because it has this small group of people called Chiefs that are fully accountable for thier division, and they are working toward making it better for the Chief that replaces the current one. A good example is the USS Vincense which was commissioned in 1985 the CO signed for the ship along with all the equipment. In 2005 I was part of the decommisioning crew. The last CO signed everything back over to the navy. Nothing was missing nothing was signed in between. Obviously things were replaced over the years, however 80% of the gear was original. That's why a SFC and above get treated better on a navy base than thier own. If your going to say a SFC is the same as a Chief the difference is beyond your comprehension. In the last 6 years I've gained insight into the Army and the differences are astounding. If you still want to say the leadership is the same explain to me why it takes a CWO to be in charge of an Army LCU and the navy does it with a E6 or E7.

The most common forms I have filled out have usually been maintenance related. I was qualified on up to QAI for sub-safe work packages. I think the commonality of such things may depend on the individual and or the command.

In my day it would have been NAVSUP form 1250, for ordering supplies. You couldn't get anything out of Supply Dep't without a 1250. I filled out far more of them than the "Special Request Chits," or anything else.

Its called PMS. And to shut any piece of equipment down? You have a paper trail to "Tag Out" that piece of equipment. And did I say that you could have 30 to 40 yellow and red tags. The good news it saves lives. The bad news is all the signatures one needs to complete the job. And I was in when there were beards allowed. And you had to fill out a 'Chit' for approval.

I dunno, but I deal with form DD 1351-2 (Travel Vouchers) constantly, to the tune of about 40 a week. That, and leave chits, check-out sheets, etc.

In my opinion the most seen and used were MAFs "Maint Action Forms". However I was a mechaninc in a Helicopter Anti-Submarine Squadron with Damage Control Petty Officer as a collateral duty, so I went through those constantly throughout the day. It has been 16 plus years since I looked at one so the nomenclature I honestly don't remember. I can do some digging though. I am sure I have an old Navy form of one type or another buried somewhere.

Everything is pretty much done on computers. Some commands still have papercopeies just in case in admin. They only time we start using actually form numbers it's when we get into supply system and personnel

Ships Maintenance Action Form (OPNAV Form 4790/2K)
Used it to get electronic parts to fix tin cans "general purpose electronic test equipment" aboard the USS Puget Sound (AD-38).
