Apologies for asking such a basic question, I've Googled around and didn't see anything that resembles what I'm looking for. The Army has a DA Form 705 that records the Fitness Test results for an individual (Push Ups, Sit Ups, 2 Mile Run) and a DA 5500 to record the body fat results. <br /><br />What is this for the Navy? I'm looking at the Navy PRT Guide but only see tables to calculate scores. Scores are tallied and entered into a database.
