What is the absolute highest rank in the US Army?

0-8 is the highest permanent rank. All ranks above that require specific Congressional approval, are term limited and require another Congressional approval to retire at that rank. Also the last rank a promotion board is held for. In theory (and in some actual practice), if a three or four star cannot find another equivalent job at the end of their two year tenure in grade, they have to retire or revert back to their permanent rank of 08. Believe it was set up this way to honor Washington to ensure that no officer would ever equal his final permanent rank of Lt General.

The President, a.k.a the Commander in Chief, has the ultimate authority, but it is no rank, therefore maintaining his civilian status. Straight from Article II, Section II, Clause I of the Constitution.

Actually authorized and implemented? 5 Star rank for Bradley and Ike. Navy had some too. Leahy and Halsey come to mind. You'll see the O-10 on the pay scale but there hasn't been one since the WW2 era. You wouldn't want to see it anymore because it would have to be really bad to need it. BTW the 5 star rank is common around the world, especially in the small potato nations.

General of the Armies(4 gold stars, treated as 6 stars) is implemented. The last persons being considered for it was GOA Bradley and GOA MacArthur. Similarly, the Navy has Admiral of the Navy, held by one person, but I'm not sure if it's just 4 stars with a title, or equal to a 6 star like General of the Armies.

An O-10 is "just" a 4-star flag officer. There are two positions higher than that, though still at the same grade.
