Where do I need to send my Federal Agency Verification for my state EMT license?

The literature online is very vague about where I should send the form. It says that I should send it to where I was trained which I assume is Ft. Sam, but I'm not sure who to address it to and I don't want it to get lost.

It must be different for my state, they want my NREMT and fed verification. Appreciate the response tho

To get your state license usually you just send your NREMT info to whatever state youre applying for your license in. I got mine through a county fire academy.
