Where I can find a copy of the volunteer memorandum for the EIB Testing?

1SG had it on disc but now its not working so we are hoping someone might have an example.

It is the volunteer statement that goes in the Expert Infantry Badge Testing packet. Need to find it ASAP so we can meet the deadline next week.
