Which DA 5690 form do I use for an interstate transfer?

For an Interstate transfer, I was told I need a DA form 5690. I went online to find it and I found at least 3 different documents, 5690,5690 A,5690 R. So which one is the one for me. I asked the OSM, but he is in a meeting. I just want to get it done.

Your question is what form do you need for an Interstate Transfer, or you need the DA form 5690 along with other forms for an interstate transfer?

There is only ONE DA 5690, Reserve Components Career Counselor Interview Record, the "A" and "R" were the old ways to note if your were able to fill it out digitally or if it was reproducible. The current version is March 2016. https://armypubs.us.army.mil/eforms/pdf/A5690.PDF
