The US Diving industry seems to have one of these about every 10 years. I was involved in the investigation of a similar one of these in the early 90s in the Sea of Cortez. 9 dead, 4 rescued. nighttime rollover due to failure of bilge pump. Unfortunate, but after a few years of no accidents the industry seems to abandum safety rules and safe seamanship like bridge watch and other procedures.
