I have a Soldiwr who is PCSing soon and has asked how he can get a driver’s badge annotated on his ERB. I know the requirements are to either have 8,000 hours or 12 months accident free, but what are the forms and how do I go about submitting it?

Literally submitted two this morning. Shoot me an email at " [login to see] and ill send you the templates I have. I cant promise that they will be "how your unit does it", but it should be a good start.

SSG (Join to see) - Usually it's just done at the unit level, Battalion or Above. The one I had for prior service was just a basic Memorandum for Record with the number of miles driven, 12 miles accident free. It was signed by the Unit Commander & uploaded to my permanent file.
