I have two, one that I’ve used for Change of Command bios and one that more of a CV (Detailed Resume) which I present to my Senior Rater and Rater at new assignments. I’m happy to share, email me at [login to see]

Not sure how much this will help if you’re active duty, I would imagine they are very similar, I used the example in the reg NGR (AR) 600-100, and googled a couple of examples. Hope this helps!
