When completing my first OER support form as a 2LT, what do I put for Thru Date of Last Completed Eval and MSAF Date?

If it is a rated period it will be the date of your academic evaluation report from BOLC. If you are starting it before BOLC as a non rated period but for a record of what you have done than it will be your commission date as a 2LT.

Concur with LT Barkman. I highly recommend including your accomplishments/contributions prior to completing BOLC.
