I have always called him Master Chief and their last name.

depends on situation - formal vs informal - formally I have addressed them as master chief of the coast guard in lieu of the common Master Chief (insert name) as is the informal.

As in calling him/her, by title, or sending a letter? The former is Master Chief, (last name), the latter is MCPOCG Steven W. Cantrell Attention: US COAST GUARD
