I've only seen it happen once... and maybe twice, if you consider Midshipmen "junior officers". We had a BMC who put on ensign, but ended up reverting to Chief, and I had a classmate who went from Midshipman to enlisted, eventually made Chief... then came back to the officer side as an ensign.
