Some people said it's Sir/Ma'am but some also say that it's Mr and Ms/Mrs

I’m used to Sir or Chief. Either one works for me. Just don’t call me late for dinner.

In the Army I believe Warrants can be commissioned to assume a command, then 'sir' is appropriate. WO1 are Mister WO2 and above merit 'Chief'.

I prefer that you genuflect in my god like presence, and either call me your holiness, why not making direct eye contact. Or Chief works too. But this advice is only valid for the Army. As the USMC doesn't like being called Chief, you can call them Sir/Ma'am, Warrant Officer for W1 or Chief Warrant Officer 2-5 (CWO for short).

Usually name first, then street address, then city, state, and zip code.

I always used Sir or Ma'am. Sorry about the above, I couldn't resist.....

CW5 Ranger Dave - Actually, we used to have a WO program, and there is some argument to bringing it back.

My youngest brother is a Chief Master Sergeant at March. He would love to see the AF bring back Warrants. Both his Dad and Brother are retired Army CWOs.
