Great question. Sometimes in war, shit happens and no one is to blame. It’s dangerous business.
But other times, threats can be mitigated. With great Intel, many dangerous situations can be planned for. Training can be conducted to plan for all contingencies. When those fail, someone is surely to blame.
In the Niger situation, I don’t know who, if anyone is to blame.
