Tragic on many levels. Individuals and rooms required 'no notice 'searches. Many have very unique hiding places. There was a case where drugs were placed under pepperoni on a pizza (not in the VA). Staff or others could have brought the drugs in...I doubt these patients are throughly assessed EVERY 15 minutes as required.

The easiest way I've seen is that nurse/doctor number 1 gives you your meds, forgets to mark it, does shift changeover and then nurse/doctor number 2 comes in and gives you your meds again.
