Unfortunate any time we lose a soldier. War is a dangerous undertaking. RIP Sgt Matt Tonroe.

"friendly fire" is a misnomer, especially when it is lethal and unintended. In Nam many citizens were also killed unintentionally. In fact more citizens were killed than combatants on both sides combined.
