I've had an instant open (main) with a severe bow tie. Opened so fast the chute ripped from front, almost to back down the center line, broke 4 A lines, 4 B lines, and 1/3 of my C & D lines. Not a fun experience! My hands were already executing cut-away, reserve deploy before I could finish thinking it through. Should have knocked me out cold and one of the 3 jumps I ever did *WITHOUT* an AAD.

Probably his first jump on a civvie rig. I suspect muscle memory kicked in and he didn't release because he's conditioned to not drop his ripcord.
