Great list and good recommendations. I never thought about question 1 (would you like a water?) as part of the interview, but I offered such to interviewees when I was in private enterprise many times. I really was just being nice.

In almost every interview, I asked:
Tell me about the best boss you ever worked for (no names please); what did you learn from him or her?
Tell me about the worst boss you ever worked for (no names please); how did you overcome the problem?
Tell me more about your part in the (job or project from their resume)? What did you like? What was the most difficult?
This job requires work outside of normal work day hours and sometimes on nights or weekends. Is that a problem? (Can't legally ask if they have kids, sick relatives, or other off-limits situations, but this question is job related and will get to the core issues if any)

Most interviewees did a good job on answering these questions. If they stumbled anywhere it was on the job or project role question. That may indicate they were not fully truthful on their resume.
