Two years later and the Phoenix VA is still horribly broken. I know I am an outsider to the VA system and improvements take time, but this is unacceptable.

It appears to always be the same locations, or states with the biggest problems. These states need to be scrutinized closely, and the higher ups need to be removed from their positions and be replaced by others who have already proven effective in similar roles in the VA system.
