No surprise that both ships were found to be at fault - 99% of collisions, that's going to be the case, because the rules of the road state that (paraphrasing) '...nothing in these rules is intended to prevent the use of good judgment in order to prevent collisions'. In other words, "Break the rules if you need to, but you'd best be right." So that said, I'm unsurprised that both are blamed. I AM surprised they're recommending charges for the Crystal officer. I didn't think they would. Good on them.
