FAA Commercial Pilot Certificate front and back
FAA Class I or II Medical - Must be dated within last 12 months
Flight log - last page showing total hours
VRA Eligibility Documentation:
Disability letter (percentage and honorable discharge) or
Member 4 DD214 with qualifying medals or dates
