To reenlist into the NG, is there a template or form to write a letter stating why I got out, why I want back in, and what I've accomplished?

I am trying to reenlist into the FLARNG. When I was discharged, I was not given a RE code or reason, and my recruiter told me that I need to write a letter to the AG FLARNG. The letter should state why I got out, why I want to get back in, and what I've accomplished and done in the time. I've been out 4 years, so i'm still in the window to reenlist. Is there a template, d-form or is it just a personally written letter. Also would anyone be willing to proofread it (recruiters, command level staff). Thank you!

You’re not given an RE code for separation from Reserve component or a DD214. That’s for separation from active service

SPC Michael Keller that’s for your recruiter to answer. An experienced recruiter knows reserve component SM receive separation orders and not DD214 with an RE code, except for periods of active duty like Basic and AIT. You were kicked out and you need a waiver. There’s no template or form, someone is going to sit back and evaluate whether or not it’s worth the ARNG giving you a second chance, based on what you’ve accomplished in the past four years

I would go back to the recruiter for a template/format, other wise use a standard personal letter format.

If you were Army National Guard, you should have been given an NGB22 when you separated, which should have the type of discharge, Honorable, under Honorable Conditions, etc, and it should have a RE Code on it also. If you don't have it, HR at state headquarters should be able to get that for you. If you are close to your old unit, the Admin may be able to pull it up for you.
