Is there anyone that knows anything about getting a surgical release form?

In 01/2016, I went to Parris Island to begin training for the Marine Corps. Half way through training I got a stress fracture in my right femoral neck (hip). After spending months in MRP I was discharged and was given the re-entry code RE-3P. I started the reenlistment process with the Air Force in June or July of 2017. After submitting updated x-rays and my medical information from Parris Island I was told I would need a surgical rease form to continue the process. I do not know why because I never had surgery. The recruiter said he contacted the liaison at MEPS and was told it was up to the people at Parris Island to get the form together. I was wondering if there was someone I could contact about getting the form moved along.

I can ask my husband. He fractured his hip in CTT selection but wasn't discharged or DQ'd, but he had to have waivers to stay in.

Also I’m pretty sure you can get this waiver at MEPS when I came back in, in 97 I had to have a waiver for my ankle and they did the waiver right there

I'll give the naval hospital a call first. I'm not sure if they will have records or not since I was never there. They brought me to a private doctor's office in Beaufort to get a cat scan of my pelvis, determined I didn't require surgery, and stuck me in a rehabilitation platoon. During that time I would go to a small rehabilitation area ran by corpsman but it was still on Parris Island.
