What does does TI class 4 on my flight physical mean?

It's probably a simply answer but I was under the impression I was doing a class 3 physical and so was the FS and when it finally came back. The FS handed me my physical packet and said I was good to go. It has the Qualified stamp on it which is good but looking into the packet I noticed it said TI; Class 4 Mil and that made me question whether or not this physical is any good. I emailed my FS but he's not answering and now on leave (my luck). Oh and also the ethnicity says I'm white...when I'm actually Hispanic which made me laugh til I realized that needed correcting.

So what does TI class 4 mean?
Amd how long would it take for them to fix that ethnicity box and send it back?
It's already been 3 months since I received this I'm not looking forward to waiting another.
