Will my wife be eligible for home care via tricare after I pass?

The answer is "It Depends".
TRICARE will keep covering family members when a sponsor dies and spouses can keep their coverage until they remarry*.
As to "Home Health Care", that depends as well. TRICARE does cover Home Health Care, but she'll have to have pre-authorization from them for it. See https://www.tricare.mil/CoveredServices/IsItCovered/HomeHealthCare for more details.
