A MEB/PED will be started if the condition warrants one. If it was diagnosed below the retention threshold one might not be automatically started. The commander can request one be started as well. What I found that helped was Yoga and pilates. Both strengthen the core more than just crunches do. Since I started doing that, I have had very, very few problems. Mostly when I lay off for a few days...
