The VA regulations and JCAHO guidelines has requirements under Environment of Care Standards that has a team comprised of Chiefs, Engineering, Environmental Management Service, Logistics, Safety, Quality Assurance, Nursing at least under the leadership of the Associate Director and they perform Environmental Inspection rounds bi-weekly. They are required to inspect each area of the facility at least twice annually. So why did it fail at Tampa and Kansas? Did we all forget about the 20/20 or 60 Minutes program from the 80's which showed similar deplorable conditions? WTF???
