Here is what I found out. So for stateside assignments Command Sponsorship does not apply so if care for your dependent is over the 100 mile radius the MTF will non recommend your dependent for travel. If your dependent is on Tricare PRIME then you are good and that does not mean you cannot take the assignment you know your dependent especially if you know that the over 100 miles radius distance would affect your child. So with that being said you can take the assignment the MTF still has to provide your dependent care and the MTF will reimburse you for travel to the see the specialist.

For example my daughter has a blood disorder and needs to see a hematology the hematologist is in Dallas which about 117 miles away. She has never been hospitalized or had any complications. So I took my assignment. She has been fine and I am working on getting her referral to be seen by her specialist now.

Administratively your dependent will still be on your orders there will just be a note stating nonrecommended by medical which means the you will have to cover their travel cost but most stateside assignments people chose to drive to the next duty station anyway so basically that travel cost wouldn't be a financial burden especially if you have other dependents.
